Hospital suspends heart surgery after child deaths

Four children died in Oxford hospital following cardiac surgery - the deaths are to be investigated.
Will they ask the following questions?

Question 1
Could the cause of the child's heart defect be a medication the mother was taking during pregnancy?

Question 2
Is the fact there are only 12 anaesthetists in the country qualified to do pre-surgery tests for allergies to anaesthetics, leading to avoidable deaths?
